Before your first appointment: Please print and
complete the forms below and bring them to your first appointment. Please also print the cancellation policy found below. These
will be used as our guide for your future sessions. Feel free to attach
additional pages as necessary.
Fees: The per session fee is $100, payable by cash or check. Checks should be made out to Eve's Haven LLC. There are limited sliding scale fees available, please discuss your financial concerns with me so we an work out something that benefits you.
Insurance: BCBM/BCN are now accepted. Please know your co-pay for your mental health benefit as well as any session limitations prior to your first appointment. If you have a different insurance, you are welcome to bill them for out of network. I will provide you receipts in order to do so and complete paperwork they might require.
Cancellation Policy: All appointments must be cancelled with a minimum of 24 hours notice. Please see the document above for a full explanation of the cancellation policy and penalties.
